Each pack holds three to eight individual capsules chosen for one goal — energy, sleep, weight and metabolism, hormone support, longevity, joints, gut health and others — portioned into thirty single-day pouches and delivered every 30 days. You tear one pouch, take it, and you are done for the day. There is no pill organizer and no counting bottles.

Packs run $74 to $104 per month, depending on how many capsules you take daily and which ingredients your goals call for. The price covers the supplements, personalized daily-dose packaging, and shipping to your door. There is no membership fee and no separate consultation charge to get started.

No. You do not need to be an established patient and you do not need an appointment to start. The questionnaire on this page collects what we need to screen your pack, and Dr. Belkoff reviews every protocol before it ships. If your answers raise something that needs a conversation first, we call you instead of shipping.

Three differences that matter clinically:

• It is screened against your medications. A retail shelf does not know you are on warfarin, levothyroxine, an SSRI, or a GLP-1. Our questionnaire rules ingredients out before they ever reach your pouch.
• A physician selects it, not a marketing team. We lead with transparently dosed single ingredients rather than proprietary blends that hide how little of the active ingredient you are actually getting.
• The dose is the studied dose. Plenty of retail products contain a real ingredient at a fraction of the amount used in the research.

That is exactly what the screening is for. Two examples of how it changes your pack:

On testosterone therapy, iron is withheld unless your labs show you actually need it — testosterone already raises iron, and adding more without checking ferritin is how people get into trouble.

On a GLP-1, ingredients that add to the same nausea, slowed gastric emptying and constipation get flagged rather than stacked on top of a medication already doing that.

We also screen for warfarin and other blood thinners, thyroid medication, diabetes medication, blood pressure medication, antidepressants, tamoxifen, liver and kidney disease, hormonal birth control, and pregnancy or breastfeeding.

Tell us, and it stops being one. If you identify as a drug-tested athlete, the questionnaire removes DHEA, Tongkat Ali, Tribulus and proprietary libido blends from your pack entirely — those appear on the WADA prohibited list and in most workplace testing programs. They are excluded outright rather than flagged with a warning, and your results page names what was left out and why.

Biotin can. High-dose biotin, the kind in most hair, skin and nail products, interferes with a number of common lab immunoassays — including thyroid and cardiac markers — and can push results in either direction. If biotin is in your pack, your results page flags it, and you should tell whoever draws your blood before any lab draw. Several other ingredients can shift thyroid medication absorption, which is why we ask.
