What Is Medical Weight Loss, and How Is It Different From Dieting?

Medical weight loss is physician-supervised treatment of the biology that makes weight come back — appetite signaling, insulin response, and the metabolic adaptation that follows every round of dieting. At Prime Body Solutions in Liberty Lake, Washington, that means a GLP-1 medication such as semaglutide or tirzepatide, prescribed and monitored by Dr. Cody Belkoff, DO, paired from the first visit with nutrition planning, protein targets, and resistance training.

Dieting and medical treatment fail for different reasons, which is why one is not a substitute for the other. A diet asks you to override hunger signals that your body strengthens as you lose weight — you are fighting your own physiology, and it does not tire before you do. Medical treatment changes those signals directly. That is what makes the habit work possible, and the habit work is what makes the result last.

How GLP-1 Medications Actually Work

01 — Appetite

They quiet the signal, not your willpower

GLP-1 medications act on the appetite centers of the brain, so you feel full sooner and stay full longer. Most patients describe it the same way: the constant background chatter about food goes quiet.

02 — Digestion

Food leaves your stomach more slowly

Slower gastric emptying extends fullness after a meal and flattens the blood sugar swings that drive afternoon snacking and late-night grazing.

03 — Insulin

Your insulin response improves

Insulin release in response to a meal becomes more efficient. Tirzepatide adds a second target, GIP, which is part of why it often suits patients with insulin resistance or prediabetes.

Is This Right for You?

This is likely a fit if…

  • You have lost weight before and watched it come back
  • Your weight is affecting your blood pressure, blood sugar, lipids, joints, or sleep
  • Appetite and cravings — not a lack of knowledge — are what stops you
  • You want a physician managing this, not a subscription box and a syringe
  • You are willing to work on nutrition and resistance training alongside the medication

This is not the right fit if…

  • You have a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2
  • You are pregnant, breastfeeding, or planning a pregnancy
  • You have a history of pancreatitis or certain gastrointestinal conditions
  • You want medication only, with no changes to how you eat or train
  • You are looking for a guaranteed number — no honest clinic can give you one

About the numbers you have seen. Published trials reported average total body weight reductions of roughly 15 to 17 percent with semaglutide and 20 to 22 percent with tirzepatide over 68 to 72 weeks. Those are trial averages across thousands of participants, not a forecast for any one person. Your history, your starting point, your dose, and how much of the nutrition and training side you actually do all move that number. We measure body composition monthly rather than just weighing you, because losing fat and losing weight are not the same thing.

Choosing Your Medication

Semaglutide vs Tirzepatide: Which GLP-1 Is Right for You?

Both medications are highly effective, and both are prescribed at our Liberty Lake clinic. The right choice depends on your labs, your history, and how your body responds — which is a decision a physician should make with you, not a website quiz.

  • Often the right starting medication
  • FDA-approved or compounded, depending on your situation

Dual GLP-1 + GIP Receptor Agonist

  • Average 20–22% body weight reduction in SURMOUNT trial data
  • Once-weekly subcutaneous injection
  • Stronger effect on insulin sensitivity

Start With Tirzepatide
  Semaglutide Tirzepatide
Mechanism GLP-1 receptor agonist Dual GLP-1 and GIP receptor agonist
Trial weight reduction About 15–17% (STEP program) About 20–22% (SURMOUNT program)
Dosing Once weekly, subcutaneous Once weekly, subcutaneous

Appetite suppression Strong Often stronger
Starting price at Prime Body $175 per month $250 per month
Typically chosen when New to GLP-1 therapy, cardiovascular risk factors Insulin resistance, prediabetes, limited response to semaglutide

You are not locked into your first choice. Many patients start on semaglutide and move to tirzepatide if the response stalls, and some go the other direction when side effects or cost matter more than maximum loss. Dr. Belkoff reviews your labs and progress at every visit and adjusts — changing medication is a normal part of treatment, not a failure of it.

Weight reduction figures reflect published clinical trial averages, not results guaranteed at our clinic. Individual results vary based on starting weight, medical history, adherence, nutrition, activity, and biological response.

The Prime Body Process

How Your Weight Loss Journey Begins

Four steps from your first visit to steady, tracked progress — all under the personal care of Dr. Cody Belkoff, DO, at our Liberty Lake clinic. Most patients can begin treatment at their first visit.

01
Day one

Free Consultation

A no-pressure visit with Dr. Belkoff, in person or by telehealth. We go through your goals, your medical history, and every past attempt that didn’t stick. You get a real medical opinion, not a sales pitch — including whether medication is the right tool for you at all.

02
Often the same visit

Your Personalized Plan

Dr. Belkoff chooses between semaglutide and tirzepatide based on your history and goals, using FDA-approved or compounded medication. You start at a low dose and titrate up over several weeks — the single biggest factor in avoiding the nausea that makes people quit.

03
If clinically indicated

Lab Work When Needed

Lab work may be recommended depending on your medical history, current medications, and what we find at your evaluation — a metabolic panel, hormone panel, or thyroid studies. Recent labs from another provider can often be used. Body composition analysis is done in the office.

04
Monthly, ongoing

Track, Adjust, Sustain

Monthly check-ins in person or by telehealth to review body composition, adjust dosing, and keep protein and training on track. Direct text access to Dr. Belkoff between visits — no one to chase, no one to wait on.

Losing weight is the easy part. Keeping muscle is the work. Rapid loss without adequate protein and resistance training costs you lean mass — which is why we measure body composition monthly instead of just weighing you.

If You Want to Use Insurance
Brand-name prescriptions sent to your pharmacy — $99/month

If you’d rather try to run a brand-name GLP-1 through your insurance, Dr. Belkoff will prescribe it and manage your care for a $99 monthly fee. The prescription goes to the pharmacy of your choice, and the medication cost is whatever your plan and copay work out to.

Be aware: most plans still exclude weight loss medication. Coverage is denied more often than it’s approved, and approval can require documentation, prior authorization, and weeks of waiting. If your plan does cover it, this is usually the cheapest route. If it doesn’t, the compounded programs above cost less and start immediately.

We’ll look at your coverage with you at your consultation and tell you honestly which way makes more sense.

How lab work is billed

We contract directly with Quest Diagnostics for discounted cash pricing and do not bill labs to insurance. In most cases that contracted rate comes out lower than what you’d pay through a plan once deductibles and coinsurance are counted — and there’s no claim, no surprise bill months later, and no waiting on authorization.

What’s not included: compounded pricing reflects the starting dose, and higher doses may carry additional medication cost as you titrate up. Lab work is billed separately at the contracted rate. We tell you what a change costs before it happens — never after.

Prime Body Solutions is a cash-pay clinic; program fees, management fees, and laboratory work are not billed to insurance. HSA and FSA funds are commonly used — check with your benefits administrator. Individual results vary and are not guaranteed.

Common Questions

Medical Weight Loss Questions, Answered

Straight answers about cost, insurance, safety, and what to expect — from a physician-led clinic in Liberty Lake, Washington.

They mimic hormones your gut releases after eating. They act on appetite centers in the brain, slow how quickly food leaves your stomach, and improve how your body releases insulin in response to a meal.

The practical result is that you feel full sooner, stay full longer, and stop thinking about food constantly. Most patients describe the “food noise” going quiet. Tirzepatide adds a second target, GIP, which is why its trial results run higher.

Semaglutide targets GLP-1 receptors. Tirzepatide targets both GLP-1 and GIP. In published trials, semaglutide averaged about 15–17% total body weight reduction and tirzepatide about 20–22%.

Semaglutide is often the right starting point and has cardiovascular outcome data in patients with established disease. Tirzepatide tends to suit patients with insulin resistance or prediabetes, or those who didn’t respond well to semaglutide. You’re not locked into your first choice — switching is a normal part of treatment.

At Prime Body Solutions in Liberty Lake, compounded semaglutide is $175 per month and compounded tirzepatide is $250 per month, with the medication included. That covers physician oversight, dose adjustments, body composition analysis, unlimited visits, and direct text access to Dr. Belkoff.

Pricing reflects the starting dose; higher doses may carry additional medication cost as you titrate up. We tell you before that happens, not after. No contracts — cancel any time.

For the medication, sometimes. If you’d like to try a brand-name GLP-1 through your plan, Dr. Belkoff will prescribe it and manage your care for a $99 monthly fee, with the prescription sent to your pharmacy and the drug cost set by your plan and copay.

Be aware that most plans still exclude weight loss medication. Denials are more common than approvals, and approval often requires documentation, prior authorization, and weeks of waiting. If your plan covers it, that route is usually cheapest. If it doesn’t, the compounded programs cost less and start immediately.

Not always. Most patients can begin treatment at their first visit. Whether lab work is recommended before starting depends on your medical history, current medications, and what we find during your evaluation.

When labs are ordered, we draw them through our contracted cash rate with Quest Diagnostics rather than billing insurance — in most cases that comes out lower than what you’d pay through a plan once deductibles are counted, with no claim and no surprise bill later. Recent labs from another provider can often be used instead.

Published trials reported average total body weight reductions of roughly 15–17% with semaglutide and 20–22% with tirzepatide over 68 to 72 weeks. Those are trial averages, not a promise about your result.

Sustainable loss generally runs one to two pounds per week. Faster than that usually means you’re losing muscle along with fat, which is why we track body composition rather than scale weight alone. Your starting weight, medical history, adherence, protein intake, and activity all move the number.

You will lose some lean mass on any weight loss protocol — that’s normal. The question is how much, and that’s largely within your control.

Adequate protein and resistance training are what protect muscle, and both are part of every program here. We measure body composition monthly rather than just weighing you, so if lean mass starts dropping faster than it should, we catch it and adjust. This is the single biggest difference between physician-managed treatment and a mail-order pen.

Under physician supervision, they’re generally well tolerated. The most common side effects are nausea, constipation, diarrhea, reflux, and fatigue, and they’re most likely during dose increases. Starting low and titrating slowly is the single best way to avoid them, which is exactly how we dose.

These medications are not for everyone. We do not prescribe them to patients with a personal or family history of medullary thyroid carcinoma or MEN2, during pregnancy or breastfeeding, or where a history of pancreatitis or certain GI conditions makes them inappropriate. That screening happens at your first visit.

A free consultation with Dr. Belkoff, in person at our Liberty Lake clinic or by telehealth. We go through your goals, your medical history, your current medications, and every past attempt that didn’t stick.

You leave knowing which medication fits, what it costs, what the first 90 days look like, and whether lab work is needed. Most patients can start treatment that same visit — and the nutrition, meal planning, and training side starts that day too, not later.

There’s no mandatory meal plan and nothing to buy from us. But this is not a medication-only program. Nutrition planning, meal planning, protein targets, and resistance training start on day one, built around what you actually eat and how you actually live.

The medication makes that work possible. When appetite quiets down, the habits that felt impossible for years become achievable — and those habits are what hold the result. Medication without them is a rental, not a change.

Plateaus are expected, not a failure. They usually mean one of three things: you’ve reached the ceiling of your current dose, protein or training has slipped, or something metabolic needs a closer look.

Dr. Belkoff reviews progress at every check-in and adjusts — a dose increase, a change in medication, or a shift in the nutrition side. Switching from semaglutide to tirzepatide is common when a response stalls, and it’s a normal part of treatment rather than starting over.

Stopping the medication does not automatically mean the weight comes back. What determines that is whether anything actually changed underneath it.

If the medication was the entire plan — appetite suppressed, nothing else different — then yes, appetite returns and so does the weight. That’s what happens at clinics that mail you a pen and check in by email.

We run it differently. From day one we work on your relationship with food — meal planning, nutrition planning, protein targets, resistance training, and the habits that make those sustainable. The medication opens a window where that work is finally possible instead of a daily fight. Patients who use that window build something that holds without the drug.

Patients who don’t do those things regain. We’ll tell you that honestly at your first visit. Some patients stay on a maintenance dose long term, and that’s a legitimate choice too — there are no contracts either way.

Yes. Dr. Belkoff is licensed in both Washington and Idaho, so patients in Post Falls, Coeur d’Alene, and Hayden can be treated without driving over for every visit.

We recommend an in-person first visit when possible so we can complete a body composition analysis, but follow-ups can be handled by telehealth, and medication ships to your door.
