Washington My Health My Data Act
Required by the Washington My Health My Data Act, RCW 19.373. This notice covers consumer health data that Prime Body Solutions collects from Washington residents through primebodysolutions.com.
Scope
Prime Body Solutions, 2110 N Molter Road, Suite 119, Liberty Lake, Washington 99019, is the regulated entity responsible for the consumer health data described here.
Under the Act, consumer health data means personal information that identifies a consumer’s past, present, or future physical or mental health status — including information inferred or extrapolated from data that is not itself health data.
This notice does not cover your medical record. Protected health information held by Prime Body Solutions as a HIPAA covered entity is exempt from the My Health My Data Act. Once you become a patient, the information in your chart is governed by HIPAA and by Washington’s health care information law, RCW 70.02, and is described in our Notice of Privacy Practices.
This notice covers the narrower set of health-related data collected through our website — for example, when a visitor who is not yet a patient submits a consultation request indicating interest in hormone therapy or medical weight loss.
Section 01
| Category we collect | Purpose, and how it is used |
|---|---|
| Health conditions, symptoms, and status you describe to us | To determine whether we can help you, to answer your question, and to prepare for a consultation. Reviewed by clinic staff and the physician only. |
| Health care services you are seeking | To route your request to the right service — hormone therapy, medical weight loss, peptide therapy, or longevity care — and to schedule the correct appointment type. |
| Measurements you volunteer, such as height, weight, or age | To assess whether you are a candidate for a program before your visit. |
| Medications or treatments you tell us you are taking | To identify safety considerations before a consultation. |
| Health data inferred from which pages you view | To understand which services visitors are interested in and to improve those pages. Used in aggregate. We do not build individual health profiles and we do not use this data for advertising. |
| Precise or approximate location data | We do not collect precise geolocation. Our analytics record only an approximate city or region derived from your IP address, used to understand which areas we serve. |
We collect consumer health data only with your consent, and only for the purposes listed above. We do not use it for any purpose you have not agreed to.
Section 02
We do not purchase consumer health data, and we do not obtain it from data brokers, advertising networks, or other third-party sources.
Section 03
| Category we share | Circumstance |
|---|---|
| Health conditions, symptoms, and services sought | Stored in and processed by the vendors that run our scheduling system, our website forms, and our messaging platform, so that we can respond to you and book your visit. |
| Contact details submitted alongside health information | Shared with the same service providers for the same purpose. |
| Aggregated, non-identifying page and interaction data | Shared with our web analytics provider to measure site performance. |
| Any category, when legally compelled | Shared with a government agency or court only where required by law, or to prevent serious harm. |
We do not sell consumer health data. Prime Body Solutions has never sold consumer health data and does not do so. Under RCW 19.373.060, selling consumer health data requires a separate signed authorization from you. We do not seek one.
Section 04
None. Prime Body Solutions is a single, independently owned clinic. It has no parent company, no subsidiaries, and no affiliated entities, and it shares no consumer health data with any affiliate.
Section 05
Under RCW 19.373.040, if you are a Washington resident you have the following rights regarding your consumer health data.
Ask whether we are collecting, sharing, or selling your consumer health data, and get a copy of it — including a list of every third party and affiliate we have shared it with, and a way to contact them.
Withdraw your consent to our collecting or sharing your consumer health data going forward.
Ask us to delete your consumer health data from our systems, including backups and archives, and to notify the third parties holding it.
We will never deny you services, charge you a different price, or provide a lower level of care because you exercised any of these rights.
If we decline to act, we will tell you why within 45 days and explain how to appeal. To appeal, reply to our decision in writing at [email protected] or by mail. We will review the appeal and respond in writing within 45 days, explaining the reasons for our decision.
If we deny your appeal, you may submit a complaint to the Washington State Attorney General at atg.wa.gov/file-complaint. We will include that link in our appeal decision.
Section 06
If we change the categories of consumer health data we collect, the purposes we collect it for, or the third parties we share it with, we will update this notice and the effective date above. We will obtain your consent before collecting or sharing your consumer health data for any purpose not disclosed here at the time you gave consent.
Requests and appeals under the My Health My Data Act go to the clinic directly.
