Section 01
Agreement to these terms
These Terms of Service govern your use of primebodysolutions.com, operated by Prime Body Solutions, 2110 N Molter Road, Suite 119, Liberty Lake, Washington 99019. By using the website you agree to them. If you do not agree, please do not use the site.
These terms cover the website. The terms of your care as a patient are set out separately in the documents you sign at intake — your consent to treat, your financial agreement, and your membership terms. Where those documents and this page differ, the documents you signed control.

Medical disclaimer
Everything on this website — the service pages, the blog, the FAQs, the symptom screener, the lab explanations — is general educational information, not medical advice. It is written for a general audience. It is not a diagnosis, not a treatment recommendation, and not a substitute for an evaluation by a physician who knows your history and has seen your labs.
Do not start, stop, or change any medication, supplement, or treatment based on something you read here. Talk to a qualified health care provider first.
Reading this website does not make you our patient. No physician-patient relationship is created by browsing the site, filling out a form, using the symptom screener, reading an article, sending a chat message, or booking a consultation.
That relationship begins only after you have been evaluated by Dr. Belkoff, we have accepted you as a patient, and you have signed our consent and financial documents.
Section 03
Who we can treat
Adults 18 and over
We do not treat minors. This website is not directed to anyone under 18.
Washington and Idaho
Dr. Belkoff is licensed in Washington and Idaho. We can only treat patients physically located in those states at the time of the visit.
Clinically appropriate
We accept patients only where treatment is medically appropriate. We may decline, and we will tell you why.
Requesting a consultation is not a guarantee that we will accept you as a patient.
Section 04
Telehealth
Established patients may complete follow-up visits by telehealth anywhere in Washington or Idaho. Telehealth is convenient, but it has real limits: we cannot examine you physically, and technology can fail. If a visit cannot be completed safely by video, we will ask you to come in.
Before your first telehealth visit you will sign a separate telehealth consent describing these limits in detail.
Section 05
Prescriptions
No prescription is guaranteed. Every prescribing decision is made by Dr. Belkoff based on your history, your examination, and your laboratory results. We prescribe when it is medically appropriate and we decline when it is not, including when a patient asks for something specific.
Testosterone is a Schedule III controlled substance. We prescribe it in accordance with federal and state law, which includes verifying your identity, documenting medical necessity, checking the prescription monitoring program, and requiring ongoing laboratory monitoring. Refusing required monitoring will end the prescription.
Some treatments we offer are prescribed off-label — meaning for a use the FDA has not specifically approved. Off-label prescribing is legal and common in medicine. We will tell you when a treatment is off-label and why we are recommending it.

Appointments, membership, and payment

| Cash-pay only | We do not bill insurance and we are not contracted with any health plan. You are responsible for the full cost of your care. We can provide a superbill on request, but we cannot guarantee your plan will reimburse it. |
| Membership | Month to month. There is no long-term contract and no prepaid package. You may cancel in any month. |
| Billing | Membership is charged on a recurring basis to the payment method you provide until you cancel. |
| Cancelling a visit | Please give us as much notice as you can so we can offer the slot to someone else. Any cancellation or missed-appointment fee is disclosed to you at the time you book. |
| Pricing changes | We may change our pricing. We will give existing members notice before a change takes effect. |
| Refunds | Services already provided are not refundable. If you believe you were charged in error, call us and we will look at it. |
Booking through our online scheduling system means you also accept that system’s own terms.
Section 07
Results are individual
Any outcome described on this website — symptom improvement, weight change, laboratory values, a patient’s story — is that individual’s result. It is not a promise or a prediction of what will happen for you. Results depend on your biology, your adherence, and factors outside anyone’s control. We do not guarantee any specific outcome.
Reviews and testimonials on this site reflect the honest experience of the person who wrote them. We do not pay for reviews and we do not offer anything of value in exchange for one.

Communications you agree to receive
When you give us your phone number or email address, you agree that we may contact you about your request, your appointments, and your care. If you opt in to marketing messages we may also send those, and you can opt out at any time without affecting your care.
How we handle those messages, including how to stop text messages, is described in our Privacy Policy.
Section 09
Using the website
Our content
The text, images, graphics, logo, and design of this website belong to Prime Body Solutions and are protected by copyright and trademark law. You may read, print, and share pages for your own personal, non-commercial use. You may not copy our content onto another website, republish it, or use our name or logo without written permission.
What you may not do
- Use the site for any unlawful purpose, or in a way that could damage or overload it
- Attempt to gain unauthorized access to any part of the site or its systems
- Use automated tools to scrape, harvest, or copy the site or its content
- Submit false information, or impersonate anyone
- Upload anything malicious
Anything you send us
If you send us a suggestion, comment, or idea about the website, you agree we may use it without owing you anything. This does not apply to your health information, which is governed by our Notice of Privacy Practices.
Other websites
We link to third-party sites such as our scheduling system, our patient portal, and reference sources. We do not control them and we are not responsible for their content or their practices.

Disclaimers and limitation of liability
The website is provided “as is” and “as available.” We work to keep the information accurate and current, but medicine changes and we do not warrant that everything here is complete, accurate, or up to date. We do not warrant that the site will be uninterrupted or error-free.
To the fullest extent permitted by law, Prime Body Solutions is not liable for any indirect, incidental, special, consequential, or punitive damages arising out of your use of this website or your reliance on information found here.
This section limits liability for the website only. Nothing here limits our responsibility for the medical care we provide, and nothing here waives any right you have under Washington law. A provision that is unenforceable will be limited to the minimum extent necessary, and the rest of these terms will remain in effect.
Section 11
Governing law
These terms are governed by the laws of the State of Washington, without regard to its conflict-of-law rules. Any dispute arising out of the website or these terms will be brought in the state or federal courts located in Spokane County, Washington, and you consent to the jurisdiction of those courts.
Section 12
Changes to these terms
We may update these terms as our practice or the law changes. The effective date at the top always reflects the current version. Continuing to use the website after a change means you accept the updated terms.
